Privacy Policy
This policy explains what information farajzmd.com collects, how the practice uses it, and the choices you have.
Last updated November 2, 2026.
The site belongs to FarajzMD Plastic Surgery, the medical practice of Matthew Farajzadeh, M.D., at 435 N. Roxbury Drive, Suite 400, Beverly Hills, CA 90210. If you have a question about this policy or about your information, write to info@farajzmd.com, call (213) 342-1106, or write to the address above.
What this site collects on its own
Most pages of this site collect nothing from you. The site has no account, no login, no comments, no newsletter, and no advertising. It uses no advertising pixels, no tracking scripts from social networks, no session recording, and no chat widget, and it does not sell your personal information or share it for advertising.
Server records. The site is served by Cloudflare. Like any web host, Cloudflare processes the address of each request, the page requested, the time, and the browser used, to deliver the site, keep it secure, and count its traffic. The site adds no analytics to its pages and sets no analytics cookie.
Cookies. The site sets no cookies of its own. Its host may set one cookie to tell visitors from automated traffic; it is needed for the site to work and identifies no one.
Typefaces. The site’s display typeface is served by Adobe Fonts. When a page loads, Adobe receives your IP address and the name of this site; it does not receive the page you are on.
Your browser. The site keeps nothing in your browser during an ordinary visit.
The booking form
The Book a Consultation page carries a request form provided by Symplast, the practice’s electronic health record and patient communication vendor, which runs its forms on a service called LeadConnector.
What you give the form. Your first and last name, phone number, and email address; and, if you choose, the procedures or concerns you would like to discuss, how you would like the practice to reach you, and a note to Dr. Farajzadeh. Please keep your medical history for your visit; the form is for scheduling.
Where it goes. What you enter goes from your browser to Symplast and into the practice’s records, where the practice uses it to reply to you, to schedule and confirm a consultation, and to keep a record of your request. It does not pass through this site’s host. If you come to the form from a procedure page, the name of that procedure is carried into the form so that it is already chosen for you; that name reaches Symplast and LeadConnector when the form loads.
What the form’s vendors record. While the form is open, Symplast and LeadConnector record technical information about the session, such as a session identifier, the page the form is on, and the page you came from, and they may set cookies on their own domains. The form also loads a check against automated traffic from Cloudflare and typefaces from Google, which receive your IP address. Symplast provides the form under a written agreement with the practice.
How the practice contacts you. After you send a request, the practice replies by phone, email, or text message, as you agree in the form, to confirm a time. Message frequency varies, and message and data rates may apply. Reply STOP to any text to stop them, or HELP for help.
Calling, texting, and emailing the practice
If you call or text (213) 342-1106 or write to info@farajzmd.com, the practice keeps your message and your contact details so that it can reply. Please do not send photographs or medical details by text or email; keep them for your consultation.
Your medical information
Information about your health is confidential under California’s Confidentiality of Medical Information Act, and the practice treats what you share, on this site or at a visit, under that law. This website is not for medical emergencies. If you have a medical emergency, call 911.
Links to other sites
The Get directions link opens Google Maps in a new tab, the research pages link to published papers at doi.org, and the Open Payments notice links to the federal Open Payments website. The map on this site is a drawing made from OpenStreetMap data and loads nothing from a map service. Once you follow a link away from farajzmd.com, that site’s own privacy policy applies.
Your choices
You may ask the practice what information it holds about you from this site, ask that it be corrected, or ask that it be deleted, by writing to info@farajzmd.com or to the address above. Requests that touch medical records are handled under California’s medical records laws, which may require the practice to keep some records for a set period.
Do Not Track and Global Privacy Control. The site sets no tracking cookies and shares nothing for advertising, so these signals have nothing to switch off.
Children
This site is for adults. It is not directed to children, and the practice does not knowingly collect information from anyone under 18 through it.
Security
The site is served over an encrypted connection, and the practice limits who may see the information you send. No method of sending information online is entirely secure, and the practice cannot promise that it is.
Changes to this policy
When this policy changes, the date at the top changes with it, and the new policy applies from that date.
